Agent requests unnecessary micro-authorizations

Symptom

The agent asks for an owner decision that is not a real human gate (e.g., "should the diagram be red or blue?").

Resolution

This is an over-stop and must be reported. The agent should make reasonable bounded decisions automatically. To unblock:

1. Reject the over-stop.
2. Re-run goal-run.ps1.
3. If the agent over-stops again, file a finding and route to PLANNER.
